Vaccine Lottery
Polk County, Iowa desires to promote and provide for the public health, safety and welfare of its citizens. To further this public purpose Polk County is offering a promotional opportunity to Polk County residents.
Beginning Tuesday, June 15, 2021 through Sunday, August 22, 2021 at midnight, Polk County will offer a vaccine lottery to help boost COVID-19 vaccination rates in the county. Individuals are eligible to register after receiving at least one dose of the COVID-19 vaccine (1 dose of either Pfizer, Moderna or Johnson & Johnson). Once individuals are fully vaccinated (2 doses of Pfizer/Moderna; 1 dose of Johnson & Johnson), they will receive a cash prize if chosen. Residents of Polk County, Iowa, are eligible to register to enter the lottery and win prizes.
Polk County will verify (To confirm) that registered participants (an individual is) are fully vaccinated, (we will) by reviewing registered participants (your) vaccination records (on) through Iowa Immunizations Registry Information System. There is no cost to receive a COVID-19 vaccine and there is no purchase necessary to enter the lottery. Individuals entering the vaccine lottery must be 18 years of age or older.
When registering for the lottery, (individuals) participants will be required (need) to provide a valid Polk County address, name, date of birth, phone number and email address. If (your) the required information is not valid, (you) the registrant will be removed from the lottery and ineligible to win. Winners will be chosen by chance. Participants are not required to be present in-person at the time the winners are determined. Winners will be contacted via phone or email.
Vaccine lottery is paid through funding from American Rescue Plan Act.
